Self Referral 

All pregnant women wishing to delivery at Basildon, Broomfield or Southend Hospitals need to access an online Self-Referral form.  Once submitted, the referral will be processed and the patient will be contacted by a midwife usually by 8 weeks, or as sooner as possible if referring late.

The maternity department is aiming to get all patients booked for care by 9+6 weeks gestation so the earlier the patient can self-refer, the better.
