Book a Routine Appointment

To book an appointment with a clinician please telephone the surgery on 01268 663140. Phone lines are open from 8am. Our telephones lines can get very busy especially between 8.00am to 9.00am each morning when patients are trying to get an urgent appointment. If lines are busy you have the option for a “call back” and you will not lose your place in the queue.

For non-urgent matters you can send an online request via PATCHS. Available 8am to 6.00pm, Monday to Friday

Routine pre-bookable appointments with any GP are available to book up to 2 weeks in advance. For continuity of care, the Practice advises patients to consult with their usual GP. 

At times of high demand, there may be a wait of one week or more for an appointment for a particular doctor. The doctors will try to see urgent cases on the same day, however this will not be a GP of choice, it will be the duty GP.

(Urgent appointments are not for Repeat Prescriptions, Fitness for work certificates or for signing forms).

Sickness Certificates (Fit Notes)

You must give your employer a doctor's 'fit note' (sometimes called a 'sick note') if you've been ill for more than 7 days in a row and have taken sick leave. This includes non-working days, such as weekends and bank holidays.

Home Visits

The medical centre is the best place for us to see anyone however, sometimes we may need to see you at home.

Every request for a home visit will be assessed by a doctor. We allocate time for visits each day but concentrate on providing surgery consultations so that more people can be seen throughout the day.

In return we ask you to:

  • Ensure that requests for home visits are done first thing in the morning, preferably before 10am.
  • Not to request a home visit unless the patient is too ill to be brought to the surgery.

This service is provided by the West Basildon PCN clinicians.

Chaperones

The surgery prides itself in maintaining professional standards. For certain examinations during consultations an impartial observer (a “Chaperone”) will be required.

This impartial observer will be a Practice Nurse or Health Care Assistant or a member of the reception team who has undergone certificated training, who is familiar with the procedure and be available to reassure and raise any concerns on your behalf. If an appropriate chaperone is unavailable at the time of you consultation then your examination may be rescheduled for another time.

You are free to decline any examination or choose an alternative examiner or chaperone. You may also request a chaperone for any examination or consultation if one is not offered to you. The GP may not undertake an examination if a chaperone is declined.

The role of a Chaperone:

  • Maintains professional boundaries during intimate examinations.
  • Acknowledges the patients vulnerability.
  • Provides emotional comfort and reassurance.
  • Assists in the examination.
  • Assists with undressing the patients, if required.

Team Members You May Have Contact With

First Contact Physiotherapist

These are experienced physiotherapists who diagnose and manage conditions affecting your joints and muscles. There is a common misunderstanding that they provide ongoing courses of physiotherapy but as their role title suggests they are instead a great first point of contact to help you understand what is causing your symptoms and to discuss and arrange the best treatment options for you. They can arrange investigations and referrals for adults as a GP would and can organise painkillers if required also.

Advanced Clinical Practitioner

These are independent practitioners that, as the name suggests, assess your medical needs to an advanced level. They can provide you with prescriptions and arrange investigations or referrals for you as a GP would, dealing with the huge range of medical issues that you as patients may be struggling with.

Social Prescriber

Social prescribers are there to help explore with you what support you may benefit from to complement your medical treatment. Their skills and knowledge is wide ranging and depending on who you see they may support you in understanding what financial support is available to you and helping you with claiming this, finding and arranging local services that you would benefit from seeing and also ensure that you continue to improve and maintain your wellbeing.

Clinical Pharmacist

They are able to assess your medical issues advising on many common health issues. Their expertise extend far beyond administering medication and they can use their experience in helping you ensure your medical conditions are managed in the best way to meet your needs.

Mental Health Practitioner

They have expertise in assessing and supporting you with any mental health condition you may have, whether it be new or more longstanding, including advice regarding starting new medication or altering any medications you may already take.

Change or Cancel an Appointment

It is important that you inform the reception staff if you are unable to attend your appointment, this will allow that appointment to be offered to another patient. If you fail to notify the Practice that you are unable to attend, you will be sent a letter informing you that you have defaulted from your appointment. Persistent defaulters maybe removed from the list.

Cancellations of an appointment less than 30 minutes before the start time will also result in a letter being sent.

Did Not Attend Policy

If a patient fails to attend a pre-booked appointment on more than two occasions in the last 12 months an informal warning letter will be sent out to you. If you fail to attend on another occasion you may be removed from the practice list in which case a formal warning letter will be issued.

 

Late for Your Appointment

Please attend your appointment on time, if you are late you may not be seen.

Out of Hours

Enhanced Access

Out of hours appointments are available from 6.30pm to 8.00pm every weekday evening and on Saturday 9.00am to 5.00pm. An out-of-hours appointment might not be at your regular GP surgery, these will be held within local practices and can be booked by contacting reception. You will be informed of the location when you book the appointment. 

Life Threatening

Call 999 or go to A&E now if:

 
  • you or someone you know needs immediate help
  • you have seriously harmed yourself - for example, by taking a drug overdose

A mental health emergency should be taken as seriously as a medical emergency.

  • Minor head injuries with no loss of consciousness
  • Fractures between shoulder to fingers and knee to toes
  • Minor burns and scalds

  • Bites and Stings
  • Sprains and strains
  • Animal and human bites
  • Foreign bodies, ear, nose, throat, skin
  • Chest injury without difficulty to breathing
  • Facial injuries
  • Firework injuries
  • Mouth injuries
  • Neck injuries
  • Minor road collisions
  • Lacerations

Children under 7 need to go to A&E.

Xray is only open until 4pm. After this time go to A&E.

Urgent But Not Life Threatening

Visit an urgent care centre if:

 
  • You have an urgent medical issue requiring on the day attention

  • You need help now, but it's not an emergency

There will be someone to provide you with advice and to direct you to a clinician if it is necessary.
