New Patient Registration
If you are a patient who is resident within our catchment area, you are welcome to register with our Practice. We now offer digital registration, please complete the registration form here: https://gp-registration.nhs.uk/F81025. Please note that we are currently processing new registrations within 10 working days. We appreciate your understanding and patience as we work to ensure each registration is completed accurately and efficiently.
If you are struggling to do this online, call the surgery where the reception team will give you a paper copy for you to fill out and return. Should you require assistance with this, one of the team will be happy to help. We are aware that our patients have differing accessibility requirements and we are dedicated to providing a high level of service to all of our community.

In addition to the registration forms, we may ask for supporting documentation of identification and your address.
We manage the number of patients we will take on each week with the aim of not swamping our service and so attempting to preserve a safe and quality service for all our patients. We accept completed forms between 2- 5pm, when we are less busy.
You will need to arrange for an appointment with our clinical pharmacist if you are on any repeat medication from your previous GP. The Practice will arrange for your medical records to be sent from your previous GP.

Updating Contact Information
If you change your address or telephone number, please inform us as soon as possible. This ensures we send all correspondence correctly.

You have the legal right to choose a GP Practice that best suits your needs. When you have found a Practice you like, you’ll have to formally register with it as an NHS patient by submitting a Registration Form to them.
