Registration process 

PreRegistrationIf you wish to register with the Practice please call reception to explain the registration process and offer you a "New Patient Pack". 

In addition to the registration forms, we ask for supporting documentation of identification and your address.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. Please make sure to fill in both the New Registration form (GMS1) and the New Patient Questionnaire. 


We manage the number of patients we will take on each week with the aim of not swamping our service and so attempting to preserve a safe and quality service for all our patients.


You can get a "New Patient Pack" from reception, alternatively you may want to print off both: the Registration form (GMS1) and New Patient Questionnaire below. Please bring both forms to the Practice along with a form of Identification and a document proving your address. We accept completed forms between 2- 5pm, when we are less busy.
